New Patient Forms

We are delighted to have your child as a part of our dental practice. We enjoy treating children and we look forward to making the visit pleasant for you and your child.

During the visit you and your child will meet our friendly staff. They will clean your child's teeth and check for decay. They will include x-rays if the doctor feels they are needed. The doctor will carefully examine the child's mouth, teeth and gums and discuss any treatment that is needed. We will make follow-up appointments for future care if necessary. Our staff will never expect a child to do anything he or she does not feel comfortable doing. We encourage total family participation in our patients' dental experience by inviting parents and siblings to accompany them to the clinic area.

Many of our parents ask how to prepare their child for their first visit. We offer an office tour for children who may have a higher level of anxiety than others. Please call our office to let us know when you would like to stop by. We want your child to enjoy getting to know our doctors and staff and be comfortable at all times.
